http://www.arrowsprings.com/

From site:
Glass knob inserts & mandrel inserts
Inserts for making glass knobs for cupboard doors, drawers, boxes and more. These " (6.35mm) outside diameter, stainless steel inserts come in two lengths. The more common " (19mm) length and a shorter " (12.7mm) length. Inserts have a hole in only one end and are threaded to accept the included, standard 1" (25.4mm) long, number 8-32 screw. Simply screw an insert onto the 12" (30cm) long stainless steel Insert Mandrel and apply molten glass to the insert.

They aren't tutorials per se, but there is some good information and inspiration in these threads:

I get hardware here http://www.dlawlesshardware.com/knob-making-bases.html and use a good 2 part epoxy for cabs for knobs. I've used the inserts too, but not directly with glass. Somebody had cracking issues with the kit thing and direct glass application. A thick or double-dipped 3/16" (pandora sized) mandrel works.

They have molds that can be used for fusing mainly but I suspect the hardware would work for the torch. I have seen a tutorial for knobs somewhere but can't remember where - will see if I can locate and post if I do....
